<p>At first glance the lesion appears to have a rolled edge and so could be a BCC, but the rest of the story does not fit. A BCC is slow-growing (2-4mm a year), non-tender and does not have an inflamed appearance. This lesion had grown quickly, AND clinically there was a tender plaque with a central ulcer and surrounding erythema, more in keeping with an SCC.</p>
<p>Dermoscopically there are no features of a BCC, instead there is central keratin (black arrow) and <strong>multiple white-yellow peripheral clods</strong> (blue arrows), which as you will see from the algorithm is in keeping with an <strong>early SCC. </strong></p>
